Position Summary
The Care Partner provides direct resident care, supports a positive physical and psychosocial environment for residents, and assists with delegated tasks and medication management.
Responsibilities
Provides direct resident care while promoting resident dignity, independence, and personal choice.
Observes residents’ physical and mental conditions and reports changes to health services leadership or Executive Director.
Administers medications and treatments according to scheduled times, following medication administration policies.
Greets residents, visitors, and staff courteously and answers phones when assigned.
Performs First Aid and initiates emergency response procedures as required.
Serves meals in the dining room, assists residents at mealtime, and assists with dining room clean up and maintenance.
Assists residents with keeping their apartments clean and communicates observed problems appropriately.
Partners with the community team to clean all common areas of the building.
Assists with laundry service for residents and building laundry.
Follows the daily schedule and the planned activity program schedule; supports and encourages residents to participate as appropriate and assists with transportation to and from activities.
Assists with tours, move-ins, move-outs, and transfers.
Attends and participates in training, in‑services, and staff meetings; completes all required training.
Performs other duties as assigned.
Qualifications
State required certification (First Aid, CPR, and Food Handlers permit if required by state).
Current First Aid, CPR, and Food Handlers permit certificates.
WORD PROCESSING skills.
Must hold state required delegation and/or training requirements and successfully complete the ADL competency demonstration.
High school diploma or GED with one or more years of personal care experience with older adults preferred or equivalent knowledge and skills obtained through a combination of education, training, and experience.
