We are searching for a

Home Health Physical Therapist

that will provide evaluation and therapeutic services in the home. Care is directed at rehabilitation to maximize patient independence and adaptation in the patient's home.

Schedule Saturday – Sunday; 8:30 am – 5:00 pm

Location Sonoma and Marin County

By the Bay Health follows all CDPH vaccine requirements for healthcare personnel.

Essential Duties & Responsibilities

Evaluates patient care needs on admission and on an ongoing basis

Plans and coordinates the treatment program based on the patient assessment and consultation with the physician

Implements the treatment plan and makes revisions as required, under the physician’s plan of treatment

Notifies physician and other appropriate agency personnel of changes in the patient's condition

Documents patient assessment and on‑going care concisely according to agency policy and standards of practice

Education/Experience

Graduate of an accredited Physical Therapy Education program

Doctor of Physical Therapy (DPT) degree

One year's experience in a health care setting preferred

Certificates, Licenses, Registrations

Current California license in Physical Therapy.

Pay Range $69.00 – $76.50 hourly
