The
Behavioral Health – Case Manager II
performs case management telephonically and/or by home visit within the scope of licensure for members with behavioral health and substance abuse or substance abuse disorder needs. Subject matter expert in targeted clinical areas such as Eating Disorders, Maternity Alcohol/Drug, Autism Spectrum Disorders, etc. Responsibilities
Respond to complex cases and specific requests. Use screening criteria and clinical judgment to assess member needs. Conduct assessments to identify individual needs and develop specific care plans to address objectives and goals. Monitor and evaluate effectiveness of care plans and modify plans as needed. Support members in accessing appropriate quality and cost‑effective care. Coordinate internal and external resources to meet identified needs and collaborate with providers. Serve as a resource to other behavioral health case managers. Participate in cross‑functional team projects and initiatives. Minimum Qualifications
MA/MS in social work, counseling, or related behavioral health field, or nursing degree with minimum of 3 years of clinical experience in social work counseling and broad experience with complex psychiatric and substance abuse treatment; or equivalent combination of education and experience. Current active unrestricted license such as RN, LCSW, LMHC, LICSW, LPC, LMFT, LMSW, or Clinical Psychologist to practice as a health professional within the scope of licensure in applicable states or territory of the United States required. Previous experience in case management and telephonic and/or in person coaching with members with broad range of complex psychiatric/substance abuse and/or medical disorders. Managed care experience required. Preferred Skills and Experience
Experience in health coaching and motivational interviewing techniques preferred. Position requires 2 days a week in designated inpatient facility both provider and member facing. Candidate must live in MA. Equal Employment Opportunity
