Summary
The Clinical Pharmacist (Formulary) functions as a critical member of the pharmacy team working to aid in all formulary matters. The duties include the provision of clinical support to the facility by assisting in hospital formulary requests and order verification - administrative support to the Pharmacy Service in education and training - as well as troubleshooting - and finding solutions to complex formulary matters.

A pharmacist who has - or has ever had - any license(s) revoked - suspended - denied - restricted - limited - or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005 - Part II - Chapter 3 - section B - paragraph 16
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria)
Grade Determinations: In addition to the Basic Requirements above - applicants must meet the following grade requirements
GS-12 Clinical Pharmacist (Full Performance Level) Experience or Education
In addition to the basic requirements - candidates must meet one of the following: 1
1 year of experience equivalent to the next lower grade level OR - 2
Completion of an ACPE-accredited Pharm.D. program
AND - Pharmacists assigned to this position must demonstrate the following knowledge - skills and abilities (KSAs): 1
Knowledge of professional pharmacy practice

Ability to communicate orally and in writing to both patients and health care staff

Knowledge of laws - regulations - and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security
4
Skill in monitoring and assessing the outcome of drug therapies - including physical assessment and interpretation of laboratory and other diagnostic parameters
GS-12 Assignment: A pharmacist in this assignment handles routine medication-related activities in accordance with local - Veterans Integrated Service Network (VISN) - and national policies and regulations
These include - but are not limited to: reviewing - interpreting - and verifying medication orders for appropriateness
processing and filling medication orders
interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care
reviewing the patient's medications - allergies - labs - and other pertinent information from the medical record to identify and solve medication-related problems
contacting providers as appropriate
documenting recommendations and interventions
providing refill extensions and partial medication supplies
taking health and medication histories
performing medication reconciliation
providing drug information
assisting in formulary management including therapeutic substitutions - nonformulary reviews and medication usage evaluations
documenting and assessing adverse drug events (ADEs)
assisting in medical emergencies
providing oversight of technical staff in all aspects of medication distribution
Preferred Experience: Three (3) to Five (5) years of experience preferred with Outpatient and Formulary Management

Duties
CLINICAL FUNCTIONS: Demonstrate expert knowledge in drug/supply formulary in relation to multiple area of clinical pharmacy practice such as endocrinology - rheumatology - dermatology - ophthalmology - etc
Assist in the development - update - and dissemination of acceptable criteria for the use of non-formulary and restricted medications and other high-cost items to ensure cost-effectiveness without compromising the overall quality of patient care
Evaluates orders for appropriateness and drug interactions - allergies - and adverse reactions - and duplicate therapy as applicable
OPERATIONAL DUTIES: Displays competency in the use of all pharmacy resources - including computer software and hardware and other technology and assisting in training/education of other pharmacy staff as appropriate
Reviews service connection for appropriate costs and payment and applies VA local standards for appropriate dispensing and filling
EDUCATION AND RESEARCH: Responsible for maintaining professional competencies of information on new drugs - new drug therapies - innovations in pharmacy practice
Serves as a pharmacy resource to provide or coordinate the provision of drug information to health practitioners working within the facility
Information is provided verbally or in writing and may be complex in nature requiring a significant amount of research
Serves as preceptor and shares responsibility for training - guidance - and evaluation of pharmacy students and/or pharmacy residents as requested
ADMINISTRATIVE SUPPORT: Assist to ensure resolution/completion of non-formulary requests within the appropriate timeframe
Works with formulary team to identify target areas - formulate and develop plans for implementation - train staff - and provide documentation to assess progress for formulary operational activities
Serves as a liaison between Pharmacy Service and health care providers regarding formulary operational activities
Performs other duties as assigned
Work Schedule: 7:00am - 3:30pm
8:00am - 4:30pm - 9:30am - 6:00pm (Rotating work schedules)- Weekend and Holidays as Needed
