Spends a majority of time in direct service activities within the Center, including one-on-one and group counseling, interaction with other team members, and ongoing assistance to participants to address social issues. Based on comprehensive assessments, provides perspective on the social, emotional, cognitive and ethnic factors that impact the creation of an individualized integrated plan of care. Accesses community and financial resources for participants and ensures continuity of care across the continuum. Conducts social work assessment and intervention in other care settings, including the participant's home, as needed.A candidate in this position will do some, though not all, of their assessments in the homes of the participants. It is a moderate to light amount of in-field work, and it's definitely far less than a home health or hospice social worker would do. This field work would be in South King County. A candidate will be reimbursed for mileage and hours driving from one work location to another. This will count as paid time worked (ex: MSW drives from PACE center to participant's home for assessment, then drives back to PACE center). It is expected that all travel will occur within the workday of 8am-5pm, which would all be paid time. Mileage will be reimbursed when the candidate is traveling from one work location to another, but the candidates commute from their personal home will not be paid. If Client does not provide a mobile phone required for the assignment, Client agrees to pay Agency monthly, for all work-related cell phone use by each Provider, up to a maximum amount of $50.00 as invoiced by Agency. Providers will be responsible for submitting monthly cell phone bill directly to Agency.
