Overview Behavioral Health Associates assist social services teams to provide supportive counseling, advocacy, and education to help patients and their families navigate mental illness, access community resources, and manage symptoms to help them remain safely in the community. This is a junior position that provides direct care as part of a team. VNS Health Behavioral Health team members provide vital client‑centered behavioral health care to New Yorkers most in need, across all stages of life and mental well‑being. We deliver care wherever our clients are, including outpatient clinics, clients’ homes, and the community. Our short‑ and long‑term service models include acute, transitional, and intensive care management programs that impact the most vulnerable populations, from children, to adolescents, to aging adults.

What You Will Do

Assist with the development of the social work assessment of patients/families

Assist in the development of a social work treatment plan and goals with emphasis on symptom management and psychiatric rehabilitation

Provide ongoing social work services and/or collaborate and refer to appropriate agencies

Provide advocacy and assistance in obtaining entitlements and community services to patients, families and other caregivers

Assist clients with understanding their mental illness by providing psycho education and assistance with symptom management

Assist staff, patients, families and other caregivers to understand third party reimbursement, entitlements and community resources

Initiate and maintain written and verbal communications according to VNS Health policy

Perform established workflow procedures

Maintain productivity sufficient to meet VNS Health goals

Act as liaison with other community agencies

Participate in related VNS Home Care activities, e.g., utilization review, staff meetings, education programs, etc.

Assume responsibility for continued professional growth

Qualifications Education:

Bachelor’s Degree in Social Work or a related human services field required.

Work Experience:

Minimum one year of social work experience in a mental health or social service setting required.

Pay Range USD $25.46 - $31.86 /Hr.
