Job Summary The Solid Organ Transplant (Cardiothoracic) Clinical Pharmacist Specialist position is responsible for providing leadership and clinical pharmacy services, including the implementation of System, Region, and Facility Pharmacy Services initiatives. The incumbent will establish and maintain clinical pharmacy programs, mentor staff pharmacists to clinical pharmacy activities, educate pharmacy residents and students affiliated with the facility, provide support to service lines, and collaborate with pharmacy leadership to develop action plans supporting department initiatives. All activities align with CSH’s mission, vision, and values.

Key Responsibilities

Promote long and short‑range planning to respond and integrate within the market, region, and systems.

Collaborate with peers to standardize clinical pharmacy systems when appropriate.

Develop, coach, and mentor pharmacy staff to maximize their scope of practice in providing clinical pharmacy services and scholarly activities across the continuum of care.

Educate medical staff on cost‑effective, quality pharmaceutical care based on evidence‑based medicine and best practices.

Participate in assigned committees relevant to knowledge, experience, and practice area.

Ensure clinical pharmacy initiatives comply with regulatory requirements.

Precept pharmacy interns and residents.

Function as a clinical pharmacist (centralized or decentralized staffing).

Serve as Residency Program Director as applicable.

Perform all duties of a registered staff pharmacist working online or onsite.

Maintain exempt status for the specialist unless otherwise defined by contract or regional standard.

Clinical Pharmacist Functions

Document clinical activities and interventions such as consults, patient teachings, and other patient‑specific information.

Interpret medication orders and/or prescriptions for drug interactions, appropriate drug selection, and appropriateness; accurately enter the order or prescription into the system, label, and dispense medications in a timely manner.

Verify and check medications prepared by other pharmacy personnel.

Communicate drug information to patients and health professionals; resolve drug‑related problems by consulting with other health care professionals to achieve appropriate patient outcomes.

Clarify unclear orders written by a licensed provider and assist the licensed provider in appropriate drug selection and therapeutic regimens.

Maintain a working knowledge of automated systems and provide basic clinical services for all age groups served, as required by policy and procedure.

Perform duties in documented accordance with regulatory and governing bodies.

Complete mandatory education from a system and department level as appropriate.

Provide patient counseling, drug information, and documentation as appropriate.

Minimum Qualifications

Bachelor of Science in Pharmacy or Doctor of Pharmacy degree.

Residency completion or three (3) years of clinical experience.

Active Texas Pharmacist License in good standing.

Board certification (e.g., BCCP, BCPS) required within 3 years of hire; incumbents not required to obtain board certification.

BLS certification issued by the American Heart Association.

ACLS certification issued by the American Heart Association.

Pay Range $49.78 – $74.05 /hour
