Summary / Objective In keeping with our organization’s goal of improving the lives of the Guests we serve, the Restorative Nurse plays a critical role in providing superior customer service and nursing care to all Guests. Under the direction of the Director of Nursing, the Restorative Nurse LPN implements and directs the facility’s restorative nursing program with the goal of helping Guests reach and maintain their full mobility potential.

Essential Functions

Develop, implement, direct, and evaluate the facility’s Restorative Nursing Program.

Consult with the facility’s interdisciplinary team regularly to develop and maintain restorative care standards.

Ensure compliance with applicable laws, regulations, and national restorative nursing standards and requirements.

Participate in the admission process by meeting with new Guests and providing a clear overview of the restorative program.

Participate in discharge planning and implementation of Guest care plans and assessments.

Perform administrative tasks such as charting, care planning, reports, and related documentation.

Complete assigned MDS portions accurately and on time.

Assist with the recruitment and selection of restorative nursing staff.

Provide supervision to C.N.A.’s and all subordinate staff, checking their work to ascertain that assignments have been completed.

Complete annual performance reviews of all subordinate restorative staff and provide guidance and education to staff regarding their performance.

Provide counseling and disciplinary action to subordinate staff members as needed.

Assist with and participate in the disciplinary process of subordinate restorative staff.

Assign all C.N.A.’s and other subordinate staff to Guests, assign break times and all regular and special assignments.

Perform other nursing care assignments by working as a floor nurse when needed.

Only practice nursing procedures and tasks with the scope of licensure.

Ensure that Guest care plans are being followed and assess each Guest’s status in accordance with their care plan.

Round with physicians when needed.

Be knowledgeable of individual care plans and support the care planning process by reporting specific information and observations of the Guest’s needs, preferences, and changes.

Assure that established infection control and prevention practices and standard precautions are maintained at all times.

Follow established safety precautions when performing tasks and using equipment and supplies.

Report all hazardous conditions, damaged equipment, accidents/incidents, and supply issues to appropriate persons.

Maintain the comfort, privacy, and dignity of Guests and interact with them in a manner that displays warmth, respect, and promotes a caring environment.

Ensure each Guest receives person centered care.

Answer and respond to call lights promptly and courteously.

Communicate and interact effectively and tactfully with Guests, visitors, families, peers, and supervisors.

Attend and participate in departmental meetings and in‑services as directed.

Recognize your role as part of the Quality Assurance and Performance Improvement (QAPI) efforts of your organization.

Attend trainings to build understanding and capacity to undertake QAPI work.

Carry out QAPI roles and responsibilities as assigned.

Follow established policies and procedures in support of QAPI efforts.

Share notable opportunities for improvement with leadership and detect barriers preventing project success.

Support QAPI efforts both verbally and non‑verbally (i.e., through actions and attitude).

Participate in compliance trainings as scheduled and adhere to compliance plan.

Supervisory Responsibility This position directly supervises Nursing department staff based on scope of practice.

Work Environment This job operates in a health care setting. This role requires regular walking to various locations around the facility while pulling or pushing carts. This position works mostly in the Guest care areas as well as in occasional outside weather conditions. This role may also come into contact with patients who may have contagious illnesses.

Physical Demands While performing the duties of this job, the employee is regularly required to talk and hear. This position is very active and requires standing, walking, bending, kneeling, and stooping on a regular basis. The employee must be able to lift and move items and patients weighing at least 50 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.

Position Type and Expected Hours of Work This position is part of a health care facility that is open 24 hours a day, 365 days a year. Rotating shifts as well as holidays and work during inclement weather will be required.

Travel No travel is expected for this position.

Required Education And Experience

Graduate from an accredited nursing program.

Valid, unencumbered Licensed Practical Nurse (L.P.N.) License in the state of practice.

C.P.R. Certified.

Preferred Education And Experience

One year experience as an L.P.N. in a long-term care setting.

One year experience as a C.N.A.

Additional Eligibility Qualifications Knowledge of Federal, State and Local requirements and regulations.
