Family Practice/Primary Care Nurse Practitioner

Great opportunity in Idaho Falls, ID for a Nurse Practitioner to perform Primary Care in a clinic and Annual Wellness Visits in-home working for a private group. Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP) 2+ yrs exp as NP required - Primary Care, Family Med, Internal Med, Gerontology, or Palliative Care exp. Home Care exp is a plus Duties will include working in a clinic and performing annual wellness exams for senior Medicare patients in their home This role is a mix of outpatient in-clinic setting, in-home health visits, and occasional telehealth appointments Collaborate with Primary Care team to ensure completion of holistic patient care Must have active ID medical and drivers license, active DEA, currently enrolled in Medicare and in good standing, have HCC coding exp, and have Annual Wellness Visits experience 40 hours a week - Mon-Fri 8a-5p Salary based on experience and includes plenty of great benefits This fast growing organization delivers care to older adults in access starved communities Innovative model is led by NP's and focuses on outcomes, not volume - spend more time with patients, quality over quantity - 8-10 patients per day Please refer to Job ID.
