What Youll Do Oversee day-to-day clinical operations at High Pointe House and in skilled nursing facilities. Lead, mentor, and support a team of hospice clinicians and staff, ensuring adherence to best practices and standards of care. Monitor patient census, trends, and outcomesusing data to continuously improve care delivery. Collaborate with physicians, referral partners, and internal teams to ensure patients are identified and supported through their hospice journey. Ensure compliance with hospice and palliative care regulations, policies, and quality standards. Build relationships with referral sources and community partners to expand awareness and access to hospice services. Provide ongoing education, coaching, and performance feedback to staff, fostering a culture of compassion, accountability, and teamwork.

What Were Looking For Bachelors degree in Nursing (BSN)

required Current Massachusetts RN license 5+ years

in hospice, palliative, or complex care 2+ years

of progressive leadership or management experience Strong team-building, communication, and problem-solving skills Passion for leading a team that delivers patient-centered, family-focused care
