Overview

The WellBe care model is a Physician Led Advanced Practice clinician driven geriatric care team focused on the care of frail, poly-chronic, elderly Medicare Advantage patients. Care is provided throughout the continuum – from chronic and urgent care in the home, to hospital, to skilled nursing facility, to assisted living, to palliative and end-of-life care. The team partners with the patient’s primary care physician to provide concierge level geriatric medical care and social support in the home and to coordinate across the entire care continuum. Position

Pharmacy Outreach Specialist on the central quality initiatives team. This role acts as a liaison between patient, provider, and pharmacy to promote medication adherence and improve pharmacy quality measures, specifically the Part D STARS measures proportion of days covered (PDC). Responsibilities

Assist patients in adhering to their medication regimens and support clinicians and pharmacies in distributing medications. Pursue new medications and/or refills for patients via telephonic outreach, faxes, and coordination with prescribers. Work in a call-oriented, repetitive, and fast-paced environment; comfortable using multiple computer programs and interpreting data accurately. Provide patient education on medication adherence, formulary options, and available resources or assistance programs. Engage patients identified with medication adherence gaps and coordinate refills and related communications. Reach and maintain productivity and quality metrics; assist in targeting highest-priority patients for outreach. Contact prescribers’ offices and patients via telephone and text to pursue medications. Provide one-call resolution on all calls and maintain professionalism when speaking with patients, providers, and team members. Maintain open lines of communication with team members to address potential issues; triage calls/voicemails and resolve prescription issues as appropriate. Record and maintain member information in accordance with policies; educate patients on adherence and care management programs; encourage participation in programs and explain services. Coordinate medication refills with retail pharmacies, primary doctors, or other clinicians; obtain demographic information as needed for mail-order prescriptions. Ensure accuracy of demographic information and transfer prescriptions as requested. Contribute to reporting and record-keeping related to outreach and ad-hoc analyses; adhere to HIPAA requirements during all interactions. Qualifications

Educational/Experience Requirements

High school diploma or equivalent Minimum of 3 years of experience in customer service or outreach; or any combination of education and experience providing an equivalent background Pharmacy Technician certification or formal training required Required Skills and Abilities

Ability to navigate multiple computer systems; proficient on the phone when engaging with patients, pharmacies, doctors, and insurers Call center experience; strong MS Office and Google Docs skills Ability to analyze large data sets and make sound business decisions Excellent oral, written, and interpersonal communication; problem-solving and facilitation skills High attention to detail and strong organizational abilities Patient-centric mindset focused on high-quality care Work Conditions

This position may be conducted in a variety of settings (office environment typical). Moderate noise level; prolonged sitting; ability to lift and move up to 25 lbs; standard office equipment usage; requires concentration and effective communication via telephone or computer.
