Advanced Practice Clinician (APC) You will be part of a transformative program designed to provide personalized care and support to our patients, including those with complex and chronic needs, all in the comfort of their own homes.
Responsibilities
Deliver high-quality clinical and home-based patient care while promoting optimal health outcomes and providing cost-effective solutions that adhere to Alignment Health’s policies, state and federal regulations.
Assess, develop, and coordinate care options tailored to the needs of our members, typically 65 and older.
Provide focused, compassionate care during home and virtual visits ranging from 45 to 60 minutes, including initial visits, annual check-ups, and additional support as needed to address medical and social challenges.
Qualifications
Master’s degree from an accredited NP Program or PA program.
Active California state Nurse Practitioner or Physician Assistant license.
Active RN License and furnishing number (must upon hire) NP only.
Active Nurse Practitioner Board Certification/Physician Assistant Certification.
NPI Number, DEA, Valid BLS.
Valid California driver license and current automobile insurance.
Preferred Experience
One (1) year of prior clinical or home care experience.
Previous EMR experience preferred.
Experience in care of older adult (geriatric) patients preferred.
Benefits
Yearly bonus up to 7.5% of annual base salary.
Restricted Stock Unit (RSU) grant up to 15% of annual salary.
Continuing education reimbursement up to $1,500 per calendar year + 3 CME days.

Pay Range: $113,332.00 - $169,999.00.
