We specialize in diabetes management, chronic disease care, nutrition, and patient education. Our mission is to empower patients through medical care, lifestyle guidance, and preventive health strategies.
Our providers travel to patients’ homes in Plano and surrounding cities, using a company-provided vehicle (with gas included) and medical tools and supplies to deliver high-quality, in-home healthcare.
Position Overview We are seeking a Nurse Practitioner (NP) or Physician Assistant (PA) passionate about senior care, diabetes management, and lifestyle education. The ideal candidate is compassionate, self-motivated, and eager to provide exceptional care outside of a traditional clinic setting.
Responsibilities Drive the company-provided vehicle to patients’ homes in Plano and nearby areas to deliver care Conduct new and follow-up office visits, annual wellness visits, preventive counseling, transitional care management, and special preventive services Prescribe, adjust, and refill medications according to clinical guidelines and patient needs Provide education and nutrition counseling focused on chronic disease management, diabetes, and preventive health Collaborate with physicians and the care team to create individualized treatment, nutrition, and education plans Document all patient encounters accurately and maintain compliance with clinical standards Build trusted, long-term relationships with patients and families
Qualifications Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in the state of Texas Experience with diabetes management, chronic disease care, nutrition, and patient education preferred Strong communication, teaching, and interpersonal skills Able to work independently and travel locally in Plano and surrounding areas Spanish language proficiency is a plus Commitment to delivering compassionate, patient-centered care
