Healthy Communities — Public Health AmeriCorps program (West Region: Stamford, Norwalk, Danbury, Meriden, Waterbury, Bristol) Overview

Healthy Communities is CHC's Public Health AmeriCorps program designed to create cohesion in vulnerable communities and promote healthcare and support services through outreach, awareness, linkage to care and support. The program identifies the needs of communities and establishes strategic plans to address them. Service delivery addresses public health challenges and makes healthcare and support services accessible through flexible delivery. AmeriCorps members conduct community outreach, education, patient engagement, and link patients to CHCI care and referrals to community-based assistance. Members also participate in statewide and local activities promoting health awareness and services. Responsibilities

Conduct monthly patient health education workshops; monitor pre-post surveys; complete data entry for workshops and surveys. Track and document all completed patient engagement activities in patients' electronic health records. Serve as a “Super User” on retinopathy cameras to train and monitor site users. Coordinate with Business Intelligence (BI) to identify patients with diabetes who have not had a retinopathy screening. Schedule patients for retinopathy screening and other appointments to aid in diabetic care. Perform retinopathy screening and patient education for high-risk patients at in-person appointments. Provide outreach and care coordination to eliminate barriers to retinopathy screening. Document screening results and complete referrals and telephone encounters (TE) to facilitate follow-up. Assist in acquiring documentation of outside exams to comply with PCMH+ goals. Coordinate with Population Health Program Manager and Retinopathy Manager to identify other high-need areas for screening during visits or calls. Create opportunities for additional engagement with high-risk patients. Promote outreach with high-risk populations at regional sites, including promotional content and patient education materials/pamphlets. Participate in outreach activities, community events, and efforts to promote care and services to patients in need. Work with the Quality Management (QM) team to identify weaknesses and help design processes for routine retinopathy screening. Participate in Population Health, Clinical Team, and Nurse Manager meetings as directed by supervisor. Participate in Performance Improvement Committee meetings and support follow-up on retinopathy screenings and diabetes action items. Qualifications

High school diploma or equivalent required. Proficiency in Microsoft Office and internet-related applications. Excellent time management and organizational skills. Excellent oral and written communication skills. Ability to problem solve and remain calm during crises. Successful clearance of all required criminal history checks (NSCHC). Ability to travel between CHC sites and within the state. Preferred

Associate’s degree in public health or related field. Experience in patient care and engagement. Experience and/or understanding of data analysis. Organization Information

Part-time (10-month AmeriCorps term: November 3, 2025 through August 28, 2026). Minimum 40 hours/week, up to 1700 hours in the service year. Benefits & Terms
