Consumer Services Coordinator (Case Management) for Riverside Adult West

The Consumer Services Coordinator (CSC) is responsible for coordinating the services and supports that are available to persons with developmental disabilities and their families in accordance with the Lanterman Developmental Disabilities Services Act. This position provides services to consumers in the Riverside Adult West area, including driving to various locations. Mileage reimbursement for business travel is paid out on a monthly basis. Hourly Range:

SIGN-ON BONUS! $250 sign-on bonus after 90 days of employment with good standing. $500 sign-on bonus after 6 months of employment with good standing. Essential Duties And Responsibilities: Provide case coordination services for consumers. Utilize Person-Centered Planning strategies to create and execute the Individual Program Plan (IPP). Evaluate the consumer’s progress and plan as identified in the IPP. Provide advocacy services to consumers and their families. Seek out and effectively utilize generic resources on behalf of consumers and their families. Complete individual or family crisis intervention and document activities. Complete Special Incident Reports on a timely basis. Coordinate consumer assessments and placement activities. Ensure Medicaid Waiver standards are maintained and documentation completed on a timely basis. Minimum Position Requirements: Bachelor’s degree and one year of experience in intellectual/developmental disabilities, social work, or a related field. OR Master’s degree in social work, psychology, or a related field. Bilingual preferred but not required. Ability to establish and maintain effective working relationships with others. Maintain good attendance and punctuality. Ability to follow oral and written direction. Good verbal and written communication skills. Full use of an automobile, possession of a valid California driver’s license, and liability insurance. We are proud to be an EEO employer.
