Graham Healthcare Group is seeking a Home Health Speech Language Pathologist for a part-time position in Warrenville, Illinois. Responsibilities
Collaborate with the care team to provide management and delivery of patient care plans in a home health setting. Support patients in Lombard, Glen Ellyn, Glendale Heights, Carol Stream, Addison, Wheaton and surrounding areas, including travel to patient locations (~20% of days). Qualifications
Master’s degree from a speech and hearing program certified by the American Speech-Language-Hearing Association to practice in the state where you work. License and certification as a Speech Therapist, issued by the state in which you will work. Minimum of one year healthcare experience. Current driver’s license and ability to spend ~20% of your day driving to/from patient locations. Commitment to consistently meet critical deadlines for charting. Ability to self-manage time and schedule. Experience with tablets, mobile phones and EMR software. Benefits & Rewards
