, and the County is working to revise and expand our response to help all who are affected. We are looking for qualified and passionate individuals to help us in the mission of addressing issues like housing, mental health, and substance use. The Department of Mental Health is seeking qualified candidates to fill emergency Supervising Community Health Worker vacancies related to the homelessness crisis. Under the emergency order, applicants who meet the requirements may be hired for an initial period of up to 90 days, with an opportunity for permanent County employment. Before the end of your initial work period, you will be assessed on your work performance. This assessment will be weighted 100%. Those who successfully pass the assessment will be considered for permanent appointment to Supervising Community Health Worker. Responsibilities: Supervises the work of a team of Senior Community Health Workers and Community Health Workers in providing direct health, mental health, or social services to clients and patients. Assigns service referrals received from social workers, eligibility workers, medical case workers, and other professional staff. Discusses challenging cases with staff and makes recommendations on the best course of action. Leads the development of independent living skills for clients/patients. Makes recommendations to management for improved work methods. Identifies gaps within community programs and recommends solutions. Serves as an advocate for client/patient access to County and community resources. Assists in planning, developing, implementing, and evaluating community health programs. Identifies staff training needs and provides or coordinates training and coaching. Reviews staff documentation for compliance and provides suggestions for improvement. Serves as program representative to internal and external partners. Supervises support services to facilitate recovery and resilience. Interviews prospective personnel and makes recommendations concerning their employment. Carries a caseload that includes more complicated client/patient assignments as needed. Minimum Requirements: TRAINING AND EXPERIENCE: Option I: Three years of experience at the level of Senior Community Health Worker or similar experience at a public or private community-based organization, clinic, hospital, or health system. Option II: Five years of experience at the level of a Community Health Worker at a public or private community-based organization, clinic, hospital, public health organization, or health system. LICENSE: A valid California Class C Driver License or the ability to utilize an alternative method of transportation to carry out job-related essential functions. PHYSICAL CLASS:
Physical Class II – Light: This class includes administrative and clerical positions requiring light physical effort. Special Requirements Information: Senior Community Health Worker is defined as one who assists in the planning, development, implementation, and evaluation of community-based programs. Community Health Worker is defined as one who assists professional staff in providing direct health, mental health, or social services. EXAMINATION CONTENT: Under the emergency order, applicants may be hired for an initial period of up to 90 days, with an opportunity for permanent County employment.
