Oncology Nurse Navigator (ONN)
role involves guiding patients through their journey, from pre-diagnosis through treatment and survivorship or end of life. Collaborating with a multidisciplinary team, including PCPs, surgeons, oncologists, and others, the ONN supports patient access to care, makes referrals (financial counseling, nutrition, social work), and facilitates communication among care providers. This position is based at the John Theurer Cancer Center @ Jersey Shore University Medical Center. It requires 4 ten-hour shifts per week, with no weekends. Preferred candidates will have at least 2 years of Oncology/Leukemia/BMT experience. Responsibilities
Coordinate outpatient care for oncology patients, supporting them from pre-diagnosis through survivorship or end of life. Build and maintain professional relationships with medical and surgical teams involved in patient care. Communicate effectively with physicians and staff to outline navigation programs and resources. Ensure timely transmission of information to the care team. Facilitate access to services, track patient flow, document assessments, and generate activity reports. Assess patient needs and connect families to educational, financial, psychosocial, nutritional, and clinical resources. Collaborate with the research department and participate in tumor board meetings. Develop survivorship care plans for eligible patients, discussing these plans with patients and providers. Participate in performance improvement activities and departmental committees. Represent patient navigation services in the community, collaborating on outreach and marketing efforts. Perform other duties as assigned, adhering to organizational standards. Qualifications
Bachelor of Science in Nursing (BSN) required for hires on or after March 2024. Minimum of two years of nursing experience. Excellent communication skills. Proficiency with computer systems such as Microsoft Office or Google Suite. Preferred Qualifications
Oncology experience is preferred but not required. Licenses and Certifications
