Clinical Pharmacist Evenings – HCA Florida Fort Walton-Destin Hospital
We are looking for a dedicated Clinical Pharmacist to join our evening shift team, providing high-quality patient care and supporting our pharmacy operations.

Responsibilities
Interpret orders and transcribe or verify them accurately and efficiently into the pharmacy computer system.
Accurately compound and dispense pharmaceuticals.
Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.
Sustain the hospital formulary, minimizing nonformulary procurements and utilizing therapeutic interchanges to promote rational drug therapy selection.
Provide accurate and timely medication information to healthcare staff and patients.
Assist with clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as defined by Pharmacy Leadership.
Document all clinical activities and interventions accurately and completely.
Complete notification reports regarding medication errors and adverse drug reactions.
Effectively supervise pharmacy operation activities and provide support to the whole pharmacy team.
Qualifications
B.S. or PharmD from an ACPE accredited School of Pharmacy.
Minimum of one year of hospital experience preferred.
Active pharmacist license in good standing with the Florida State Board of Pharmacy.
