8:30 AM – 5:30 PM Home Visits Per Day:

Typically 3–4 homes, depending on patient load On-Call Requirement:

None on weekdays

Job Scope:

Make home visits to provide nursing care to terminally ill patients

Manage care plans and coordinate with doctors and social workers

Support patients and families with end‑of‑life care needs

Key Requirements:

Must be ICU- or Palliative Care-trained

Home care experience preferred
