The Role You’ll Contribute
The Clinical Pharmacist supports the mission of the department, hospital, and health-system by preventing, identifying and resolving medication use issues to assure patient safety, improve patient satisfaction, optimize outcomes and minimize cost. May work in centralized and decentralized areas. Performs all functions of a registered pharmacist including dispensing, clinical interactions with patients and healthcare team, and supervision of technicians. Responsibilities
Pharmacy Operations — participates in central and satellite pharmacy areas, manages medication inventories, supervises technicians, and oversees medication automation and computer systems. Pharmacy Practice — medication order review, patient monitoring, drug dosing, pharmacist consults, medication safety, and participation in stewardship programs. Clinical Pharmacy Services — direct patient care programs, collaboration with providers and pharmacy staff, and delivery of patient-centered care. Teamwork and Collaboration — ensure continuity of care across shifts and practice areas; work with the healthcare team with patient-centered communication. Quality Improvement and Compliance — supports QI initiatives, regulatory compliance, and accreditation processes; participates in practice advancement and research as assigned. Value You’ll Bring
We are seeking a dedicated Ambulatory Clinical Pharmacist specializing in specialty medications across neurology, rheumatology, gastroenterology, and cardiology. Responsibilities include managing the specialty medications program, providing comprehensive medication reviews, developing personalized therapy plans, coordinating with physicians, nurses and other pharmacists, educating patients, monitoring progress/adherence, and staying current with specialty medication developments while ensuring regulatory compliance. Qualifications
Education And Experience Required
Bachelor of Pharmacy, Doctor of Pharmacy, or other advanced pharmacy degree Education And Experience Preferred
PGY1/PGY2 (Ambulatory Care) Pharmacy Residency OR Three years of clinical pharmacy experience Licensure, Certification Or Registration Required
Applicable State Pharmacy License BLS Licensure, Certification Or Registration Preferred
BLS, ACLS, PALS and/or NRP may be required for some facilities based on department requirements. Refer to hiring manager for needs.
