About the Team Central Maine Medical Center (CMMC) in Lewiston is the flagship facility of Central Maine Healthcare. CMMC is a 250-bed, not-for-profit, Level III Trauma Center, offering comprehensive inpatient and outpatient services. Our rehabilitation facilities are part of an all-in-one orthopedics unit designed to support recovery and help patients return home sooner.
Responsibilities The Case Manager of Rehabilitation coordinates patient access to care by responding to inquiry calls, facilitating assessments, and ensuring appropriate referrals within the community or facility. Responsibilities include:
Conducting or overseeing comprehensive assessments to identify medical, behavioral, and psychosocial needs, including screening for emergency conditions.
Collaborating with the clinical team and on-call physicians to ensure timely and effective treatment planning for individuals with substance use disorders, dual diagnoses, or other psychiatric conditions.
Providing therapeutic support, educating patients and families to promote engagement in care, and managing admissions and pre-certification with external payers.
Qualifications
Master's degree in social work or counseling required.
RN licensure may be used in place of a master's degree.
Prior experience in a psychiatric healthcare facility working with adult and geriatric patients preferred.
Strong clinical assessment skills.
CPR and Handle with Care certification within 30 days.
May be required to work flexible hours and overtime; ability to meet typing keyboard expectations. With proper credentials, Specialist may be required to complete vitals check on patients.
