Provide primary and family-based evidence-based care including accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning Perform administrative duties such as opening/closing the clinic, inventory, patient follow-up, verifying insurance, and maintaining a clean environment Manage patient appointments and routine examinations to ensure positive health outcomes Discuss healthy lifestyle choices, complete diagnostic tests, and coordinate with other healthcare professionals for a comprehensive plan of care Encourage active participation in healthcare through services such as health screenings, behavioral health and wellness, and chronic condition management Required Qualifications

Minimum of one year of experience as a Nurse Practitioner; experience in Family medicine or primary care preferred Active Family Nurse Practitioner license in good standing Holistic, evidence-based care including education and treatment for pregnancy prevention, STI prevention and safer sex practices Order and administer immunizations and injections (including but not limited to COVID-19, influenza, TB, Depo) Minimum of two years of medically relevant experience or equivalent Complete DOT certification within 30 days of employment Proficiency in additional languages (e.g., Spanish) depending on market Basic Life Support (BLS) certification with CPR and AED Pass a FIT test and wear a respirator when required Education

Master\'s Degree level Family Nurse Practitioner program with current national board certification and state license to practice in the Advanced Practice Nurse role In select states, Master of Science in Physician Assistant Studies with current national board certification and state license to practice (or equivalent experience with a Bachelor of Science in Physician Assistant Studies) with licensure Preferred Qualifications

EPIC experience preferred Pay Range

The Typical Pay Range For This Role Is $87,035.00 - $187,460.00. This range represents the base pay for the job grade and the actual offer depends on experience, education, geography and other factors. This position is eligible for CVS Health bonus, commission or short-term incentive programs in addition to the base pay range. Benefits
