Job Overview

This .8 FTE position is part of the Triage & Transition Team, which provides care across various Emergency Department settings in person and via Telehealth. The role is primarily on-site at MHealth Fairview Ridges Hospital, Burnsville with focus on collaborative work within the emergency department. The clinician will engage in crisis assessments, therapeutic interventions, group therapy, while also developing and directing patient care and disposition plans. Building strong relationships with psychiatry, medical providers, and interdisciplinary teams is essential. Since 2002, the team has been dedicated to supporting youth and adults during crises, improving community health through innovation and partnerships.

Job Summary Under supervision, the clinician may

Conduct Crisis and Diagnostic assessments

Determine clinical interventions

Facilitate therapy

Provide education for patients and their families

The role involves managing treatment planning, developing transition and discharge plans, and functioning as an interdisciplinary team member. Supervision and consultation with a licensed supervisor are integral to clinical practice.

Job Expectations The clinician is expected to demonstrate clinical competence in treatment planning by facilitating and managing individualized plans that consider age, culture, diagnosis, and level of functioning. These plans are reassessed and adjusted as needed. Assessment responsibilities include completing and updating interdisciplinary tools weekly. Disposition planning involves coordinating care, providing crisis support, conducting behavioral assessments, and determining appropriate levels of care and discharge plans. The clinician will interface with government agencies, complete referrals, and communicate with community providers. Collaboration with interdisciplinary teams ensures continuity of care, and documentation must be thorough and timely. Therapeutic responsibilities include providing individual, couples, and family therapy, as well as psycho-education groups. The clinician is also expected to engage in evidence-based practice through policy development, literature reviews, and participation in research and learning activities.

Required Qualifications

Master’s Degree in Psychology, Social Work, or a closely related field

Completed clinical education and internship requirements

Licesure or provisional licensure by the Minnesota Board of Social Work as LGSW, or progress toward independent licensure as LMFT or LPCC or LICSW

Basic Life Support Certification obtained before start date

Proficiency with electronic health records and computer-based platforms

Ability to multitask effectively

Preferred Qualifications Three or more years of experience in crisis counseling or clinical therapy, and prior experience in hospital or emergency department settings.
