The Behavioral Health Consultant (BHC) provides expert integrated care to adult and pediatric patients as a member of the primary care clinic. They specialize in targeted, brief, evidence-based, solution-focused assessments and interventions geared toward functional improvement. BHCs are generalists providing high-volume, population-based services that are accessible, team-based, and integrated with medical care. This role supports the development of age-appropriate care from children through geriatrics and works with families and couples for targeted interventions in primary care. The BHC may be the first point of contact for mental health and substance use concerns and performs high-level consultation to physicians and other team members to develop clear treatment plans. The role requires flexibility to pivot between patient-facing and physician-facing activities in a fast-paced environment, with the ability to conduct brief visits (15–30 minutes) at high volume and collaborate closely with PCPs and other team members. The BHC is expected to engage in brief, solution-focused treatment and provide consultation and training to the primary care team in mental health, chemical dependency, and behavioral medicine. Essential Job Duties

Apply knowledge of biopsychosocial models for patient assessment, including use of adult and pediatric screening instruments and screening for substance use problems. Provide first-line, time-limited interventions for a broad range of physical and mental health concerns across the lifespan within primary care settings (pediatrics, obstetric populations, family medicine). Educate patients and families to support adherence to medical and behavioral treatment plans; discuss psychotropic medications with patients and communicate indications, effects, and side effects to primary care physicians as appropriate. Use Epic EHR to document and communicate relevant information to the medical team in a timely manner, including concurrent documentation during sessions and required billing/medical documentation tasks. Assess and manage suicidal/homicidal risk and concerns of abuse/neglect (child and elder), collaborate with Crisis teams, and coordinate care with emergency services as needed. Engage in brief solution-focused treatment for various mental health, substance use, and health-related behavior changes in primary care. Provide consultation and training to other primary care team members in mental health, chemical dependency, and behavioral medicine. Demonstrate up-to-date knowledge of psychotropic medications and discuss initiation when appropriate with primary care physicians. Clinical experience within the past five years is required. Other duties as assigned. Qualifications

Minimum of a Master’s Degree in Counseling, Psychology, Social Work, or PsyD; license in Oregon (LPC, LCSW, LMFT) is required. Clinical experience within an integrated care delivery system is preferred; fluency in Spanish is preferred. Knowledge, Skills, Abilities & Behaviors

Self-starter and team player with interest in systems issues and individual patient care. Ability to work efficiently in a fast-paced primary or specialty care setting and communicate effectively with patients and the medical team. Ability to work within an inter-disciplinary team and demonstrate cultural competency related to socioeconomic status, race, ethnicity, and culture in health contexts. Willingness to teach classes or groups relevant to the integrated behavioral health role (e.g., chronic pain management, diabetes/stress, insomnia). Experience and/or training with populations across the lifespan. Ability to present clinical cases for review with key considerations, assessment needs, and treatment plan recommendations. Clinical experience within an integrated care delivery system. Working Conditions

Typically a 40-hour work week; hours may vary. Fast-paced environment with potential high patient volume and associated stressors. Must be able to travel during the workday and/or cover at other locations as needed. Speak clearly, read from a computer screen for extended periods, and sit/stand as needed; travel up to 15% for training and meetings. Moderate to loud noise level based on patient volume.
