Position Title:
Full Time Float RN Case Manager Department:
Case Management Job Description: General Description: As a member of a multidisciplinary care management team, the nursing case manager is accountable and responsible for coordinating the care and services of selected patient populations. Essential Responsibilities: Responsibilities listed in this section are core to the position. Inability to perform these responsibilities with or without an accommodation may result in disqualification from the position. Comprehensively assesses patient goals: biophysical, psychosocial, environmental, discharge planning needs, and financial status. Closely monitors patient’s progress for changes in medical needs that could affect present and future medical treatment. Procures services and advocates on behalf of the patient and family with scarce resources. Works closely with third-party payers to resolve problems and issues affecting the continuity of patient care. Coordinates and directs discharge planning of patients with family members, healthcare team, and community. Reviews admission data within the designated timeframe to determine the suitability of the level of care according to ISD-A criteria and/or established guidelines. General Responsibilities: Performs other duties as assigned. Minimum Qualifications: Education:
Graduate from an accredited school of nursing required. Bachelor of Science (or higher) in Nursing from a Board Approved Program preferred. Experience:
Minimum of three (3) years of clinical nursing experience in an acute care facility. Licenses/Certifications/Registrations:
Current RN License issued by the Oklahoma State Board of Nursing or a current multistate Compact RN License (eNLC). Current Basic Life Support certification from the American Heart Association required upon hire. Knowledge, Skills, and Abilities: Knowledge of nursing practices and procedures. Strong clinical assessment skills. Knowledge of community resources available to facilitate patient recovery. Understanding of third-party payer issues regarding reimbursement and regulations. Ability to explain technical and complex information clearly to patients and families. Strong interpersonal skills for effective communication and assessment.
