About the role:
You will be part of our growingteam of Practice clinical pharmacists and pharmacy technicians. You willdevelop through team clinical supervision sessions and benefit from directmentoring with an experienced GP/ Lead Pharmacist.

Main duties of the job
Job Summary: To work as part of our practiceclinical pharmacy team, led by our senior clinical pharmacist, to: Improve patient safety, health outcomes andefficiency by providing direct, accessible and timely Clinical Medicinesexpertise by working as part of a multidisciplinary practice team. support proactive transfer of medicinesoptimisation workload from GPs and ANPs to the in-house clinical pharmacy team promote safe, evidence based, cost effectiveprescribing in line with ICB and NHS requirements provide expert pharmaceutical and medicinesoptimisation advice to practice staff and patients provide prescribing support in areas such as: ointerim and annual telephone and face to face medicationreviews and follow ups, including review of medication-related monitoringresults omedicines reconciliations (hospital outpatientand discharge letters; new patient registrations; amber drug requests) oImplementation of prescribing guidelines oMedication monitoring and safety oquality improvement initiatives in prescribingand medicines usage support medicines-related training and educationwithin the practice This role is pivotal to improvingthe quality of care and operational efficiencies so requires good attention todetail and practice procedures, as well as motivation and passion to deliver anexcellent service within a General Practice environment. Primary Duties and Areas ofResponsibility Patient facing long-termcondition and medication review clinics: See patients with single or multiple medicalproblems where medicine optimisation is required (e.g. diabetes, hypertension,heart failure), or where patients are prescribed large numbers of medicines orcombinations with identifiable risks. Review the ongoing medicine plan forthese patients, ensuring they get the best use of their medicines and actionand/or make appropriate recommendations to clinicians for medicine improvement Medicines support and advice in person or bytelephone Provide patient facing clinics or telephonesupport for those with questions, queries and concerns about their medicines inthe practice. Medicine information topractice staff and patients Respond to medicine related enquiries from GPs,other practice staff, other healthcare teams (e.g. community pharmacy) andpatients/carers with queries about medicines. Suggesting and recommending solutions. Providing follow up for patients to monitor theeffect of any changes. Unplanned hospital admissions Provide targeted support and proactive reviewfor vulnerable, complex patients and those at risk of admission andre-admission to secondary care Management of medicines at discharge fromhospital To reconcile medicines following discharge fromhospitals, intermediate care and into care homes, working with patients andcommunity pharmacists to ensure patients receive the medicines they need postdischarge. Signposting Ensure that patients are referred in a timelymanner to the appropriate healthcare professional for the appropriate level ofcare (e.g. pathology results, common/minor ailments, acute conditions, longterm condition reviews etc.) Repeat prescribing Support safe and timely repeat prescribing,repeat dispensing, electronic prescribing and medication review systems. Ensurepatients have appropriate monitoring tests in place when required. Identification of cohorts of patients at highrisk of harm from medicines through pre-prepared practice computer searches.This might include risks that are patient related, medicine related, or both. Contribute pharmaceutical advice for thedevelopment and implementation of new services that have medicinal components(e.g. advice on treatment pathways and patient information leaflets). Develop specialist area(s) of interest. Information management: Analyse, interpret and present medicines data tohighlight issues and risks to support decision making. Undertake audits of prescribing in areas directedby the lead practice pharmacist, feedback the results and implement changes inconjunction with the practice team. Implement changes to medicines that result fromMHRA alerts, product withdrawal and other local and national guidance. Implementation of local/national guidelines andformulary recommendations Support the development and implementation ofmedicines-related procedures within the practice, including appropriateacceptance of amber (shared care) drugs, medicines monitoring and recalls. Support the agreement and review of PrescribingFormularies and Protocols and monitor & address compliance/adherencelevels. Support the delivery of QOF, incentive schemes,QIPP and other quality or cost effectiveness initiatives. Education and Training: Provide education and training to primaryhealthcare team on therapeutics and medicines optimisation. Care Quality Commission Work with the general practice team to ensurethe practice is compliant with CQC standards where medicines are involved. Public health To support public health (including vaccination)campaigns. To provide specialist knowledge on all public health programmesavailable to the general public. Person Specification
