Overview Signify Health, part of CVS Health, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. Clinicians engage with people in their homes, help bridge gaps in care, and build trusted relationships to support healthier communities.
Job Details
Location: Chico, CA (local travel within licensed states as needed)
Hours: 29.5 hours per week
Time Type: Part time
Anticipated weekly hours: 29.5
Base pay range: $46.00/hr - $99.14/hr
Responsibilities
Conduct in-home health evaluations lasting about 45 minutes, including a medication and medical history review, physical evaluation, and optional diagnostic tests ordered by the health plan (e.g., diabetic eye exams, spirometry)
Use an iPad and a clinical workflow for each evaluation
Provide Signify Health services as indicated, including Diagnostic & Preventive Services products
Engage with individuals to answer health questions and connect them to appropriate care
Deliver virtual visits as needed
Travel locally and within licensed states to visit patients
Qualifications
Active, unrestricted licenses in the coverage areas
Board certification (required)
Multi-state licenses (preferred)
Able to cover Chico and surrounding area

Salary and Benefits Pay range: $46.03 - $99.14 per hour (varies by experience, education, geography, and other factors).
