Responsibilities
Educate patients on self-care involved in the diagnosis of diabetes and collect patient data to monitor health status and proactively manage the patient’s condition.
Organize diabetes self-care materials including nutrition, medication, physical activity, and glucose testing; educate patients on necessary lifestyle changes.
Demonstrate familiarity with pathophysiology, epidemiology, and clinical practice consistent with practice level.
Collect patient data regularly to monitor health status; provide nutrition therapy and monitor nutritional care for clients/patients of all ages (neonatal to geriatric).
Recognize, evaluate, and address nutritional needs based on cultural, ethnic, and religious beliefs.
Recommend improvements to health conditions based on analysis of patient data and refer patients to other healthcare providers when needed.
Apply principles of program and/or business management to support successful self-management of diabetes.
Educate patients on self-care and lifestyle changes involved in diabetes management.
Qualifications
Minimum of a bachelor’s degree in dietetics or food and nutrition from an ACEND-accredited program.
Clinical nutrition experience through an approved Coordinated Undergraduate Program or post-graduate internship.
Three years of experience as a Registered Clinical Dietitian.
Thorough knowledge of normal nutrition, diabetes self-management skills, and medical nutrition therapy.
Registered by the Commission on Dietetic Registration (CDR) and licensed as a Dietitian or Nutritionist in the state(s) of practice.
Certified by the National Credentialing Board of Diabetes Educators (NCBDE) and hold current Certified Diabetes Educator credentials.
