Position Overview Full-time Family Nurse Practitioner needed at Serene Harmony Nurse Specialists in Niagara Falls, NY. You will provide holistic care to individuals and families, addressing healthcare needs and advocating for equitable access to services. Collaborate with community organizations and implement health education initiatives to empower individuals and improve community health outcomes.
Work Schedule
Hours from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m.
Participation in community outreach events or health fairs during weekends may be required.
Approximately 40 hours per week.
Responsibilities
Conduct health assessments and screenings for individuals and families.
Diagnose and manage acute and chronic conditions, including preventive care.
Develop and implement health education programs with community organizations.
Provide culturally sensitive care and advocate for patient rights.
Coordinate referrals and follow-up care.
Maintain accurate medical records in compliance with standards.
Participate in community outreach and health fairs.
Stay updated on community health trends and evidence-based practices.
Minimum Requirements
Current Nurse Practitioner certification.
Active state nursing license.
Master's degree in nursing from an accredited program.
3-5 years of clinical experience, preferably in community or public health.
Strong interpersonal and communication skills.
Commitment to social justice and health equity.
Salary and Benefits
Salary range: $180,000-$200,000 annually, based on experience.
