Nurse Practitioner or Physician Assistant (Ozone Park/Brownsville, NY) Ennoble Care is a mobile primary care, palliative care, and hospice service provider with patients across multiple states. Our clinicians visit patients at home to provide continuity of care for those with chronic conditions and limited mobility. We offer programs including remote patient monitoring, behavioral health management, and chronic care management. We are seeking individuals who are driven to make a difference and share our motto, “To Care is an Honor.” Responsibilities

Provide high quality primary and palliative care to homebound patients in the Bushwick/Flatlands, NY region. Assess health through physical examinations; obtain and update medical histories and develop care plans. Communicate effectively with office staff and clinical operations across internal platforms. Deliver patient-centered care to a defined panel of patients. Attend quarterly medical staff meetings. Develop impressions, diagnoses, and treatment plans. Foster trust with patients, families, and caregivers during visits. Order diagnostic testing as medically necessary. Prescribe medications, supplies, and appropriate medically necessary interventions. Qualifications

Board Certified with a valid NP or PA certification in New York. Strong clinical skills to accurately assess, diagnose, and treat. Positive demeanor and strong interpersonal skills. Ability to problem solve and operate independently. 1-2 years of primary care experience with geriatric patients (hospice experience preferred but not required). Benefits and Compensation

A flexible schedule that enables patient visits at times that work for you. Autonomy with support from our in-house clinical team. Opportunity to grow into leadership roles. Salary range: $105,000 - $160,000+ (depending on experience and other incentives). Benefits (Full-time employees)
