Position Information
Location: Holmdel, NJ Facility: CareOne at Holmdel Practice Setting: Short Term Rehab, Long Term Care Essential Duties And Responsibilities
Provides and directs resident rehabilitation care within generally accepted practice methods, meeting center, state, federal, and accrediting standards. Evaluates residents’ needs and establishes effective and efficient treatment plans for assigned residents. Manages and coordinates supervision of treatment plans with all staff and other disciplines as indicated. Participates in screening of all residents according to directed schedules and practices. Provides feedback to the designated supervisor on clinical and rehabilitation issues. Participates in center-directed processes and services. Performs necessary clinical documentation to meet requirements from center, state, federal, and accrediting agencies. Maintains complete, accurate, and timely billing records to meet requirements. Participates in in-service training programs as requested. May travel to other facilities to provide services on a limited basis. Position Requirements
Master’s or Doctorate Degree in Physical Therapy. Current and active Physical Therapist license in the state of operation. Prefer experience/knowledge in older adult rehabilitation. Prefer knowledge of governmental and regulatory guidelines in designated healthcare settings. Why CareOne?
