Are you a compassionate physical therapist looking to improve the quality of life in your patients? The Physical Therapist provides physical therapy services to patients as prescribed by the attending physician. Qualifications
Graduate of a physical therapy school approved by the American Physical Therapy Association; or the Council of Medical Education of the American Medical Association in collaboration with the American Physical Therapy Association; or the Council of Medical Education of Hospitals for the American Medical Association. Licensed or registered by the state of California. Preference of at least one (1) year of institutional experience. Valid CPR certification, driver\'s license, and auto insurance. Responsibilities
Partner with the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests, and establish goals based on the client’s current level of functioning and potential for improvement. Guide and instruct patients in prescribed therapeutic activities to relieve pain and restore function to maintain maximum performance. Record and report to the physician and home health office staff the patient’s reaction to the therapy program and any changes in condition. Train patients, families, and caregivers in the proper use of prosthetic, orthotic, and assistive devices (cane, walkers, wheelchairs, etc.). Instruct patients, families, and caregivers in the client’s total physical therapy program and provide handouts and instructional content. Prepare assessments, develop plans of care, write clinical/progress notes, and incorporate information into the patient’s record. Prepare periodic evaluations and summary reports of the patient’s condition. Follow the Plan of Care. Communicate effectively with all members of the care team and report to the physician, Director of Nursing and designated Nursing Supervisor. Notify the Home Health Agency in a timely fashion of absences due to illness, emergencies, or other leaves that affect agreed-upon services. Participate in discharge planning activities and complete the physical therapy discharge summary within agency policies. Functional/Special Requirements
Must be able to read 12-point font or larger type. Must be able to hear and speak clearly. Must be able to stoop and bend. Must be able to carry equipment weighing up to 10 pounds and navigate stairs. Must be able to travel to prospective clients’ residences. Pay details
This position is paid by the visit. Compensation for routine visits ranges from $65.00-$70.00 per visit; evaluations range from $80.00-$100.00 per visit; and admissions range from $120-$140 per visit. Actual pay rates vary by visit type, experience, location, and years of experience. Seniority level
