Overview Heart Hospital - The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others.
Responsibilities
PATIENT CARE: Delivers patient care by applying the nursing process and professional practice model, based on theory, research, evidence-based practice, and organizational standards.
Assesses and evaluates patient care needs and applies critical thinking in patient care management.
Integrates assessment and intervention skills in the delivery of nursing care.
Collaborates with the interdisciplinary healthcare team in plan development, implementation, and evaluation of care.
Prioritizes all aspects of patient care including teaching, rounding, coaching, planning after hospital care, and delegation as appropriate.
Educates patients and caregivers about plan of care, transition of care, health promotion, and disease prevention.
Communicates effectively with patients, families, and the Interdisciplinary Patient Care Team.
Manages assignments within the Care Delivery Model of Modified Primary Nursing and allocates resources effectively.
PATIENT EXPERIENCE: Role models Relationship Centered Care and supports actions such as Admission Welcome, Bedside shift Report, and purposeful rounding.
Considers patient values, preferences, and cultural diversity in all aspects of care.
Maintains therapeutic relationships with patients and families; uses evidence-based practices to understand patient perceptions of care.
Initiates service recovery for concerns and escalates as needed.
Demonstrates caring, respect, compassion, empathy, and active listening.
Supports staff use of evidence-based practices.
Conducts Purposeful Hourly Rounding addressing the 4 Ps.
QUALITY AND SAFETY: Practices safe, patient-centered care; identifies and escalates barriers to improvement.
Demonstrates clinical skills and knowledge for quality and safety; participates in improvement projects and nursing research.
Implements high-reliability principles to identify and avoid high-risk behaviors; documents nursing practice to support quality initiatives.
Escalates potential safety hazards according to the institutional chain of command.
OPERATIONS: Performs charge nurse duties; coordinates supplies and inventory; ensures resource adequacy for patient care.
Documents accurately and thoroughly in compliance with regulatory and standards of care.
Facilitates patient throughput on a shift basis.
PROFESSIONAL DEVELOPMENT: Advances clinical competence and maintains a professional image; engages in lifelong learning and shared decision making.
Acquires knowledge related to role, patient population, and local/global health needs; participates in councils and unit initiatives.
Skills: NYS RN License; BLS, ACLS, PALS; 2 years of cardiology, tele, procedural experience; minimum 2 years RN experience; 1 year travel experience for Travel positions (not required for Per Diem); EPIC experience highly preferred; Schedule 3x12.
Education: BSN preferred; ADN considered with relevant experience and/or active BSN program matriculation.
