The RN serves as a consistent care coordinator through out the continuum of care for patients entering or within our health system, assessing the physical, psychological, and social needs of the patient, and improving the care experience for the patient and their family. It is responsible for the nursing care of patients in a clinical/office setting and coordinates communication with providers on behalf of the patient. Assesses the physical, psychological, and social needs of the patient, and improves the care experience for the patient and their family. Creates a seamless flow through the health system to facilitate the patient’s care. Develops and coordinates education, resources, and activities that provide understanding and support for patients and families. Maintains reliable systems to document, track and monitor patient data. Performs clinical procedures within the scope of nursing practice and clinical training. May perform telephone duties within the scope of RN role. Performs other job functions as assigned or requested. Education Qualifications
Associate\'s Degree Nursing – RN Preferred: Bachelor\'s Degree Nursing – RN Experience Qualifications
Minimum of 6 months RN experience Preferred: greater than 1 year experience in a similar clinical setting Skills and Abilities
Knowledgeable of system’s health resources to navigate the patient to appropriate care. Excellent verbal and written communication skills, problem-solving abilities, and conflict resolution. Strong computer skills. Must be a model of customer service and patient care, with sensitivity and compassion. Licenses and Certifications
American Heart Association Basic Life Support (BLS) Registered Nurse – Tennessee Board of Nursing Preferred: Certification in clinical specialty Supervision Provided by this Position
There are no supervisory roles assigned to this position. Physical Demands
Climbing, pushing, standing, hearing, walking, reaching, grasping, kneeling, stooping, and repetitive motion. Must have good balance and coordination. Medium work – exerting up to 50 lbs occasionally and up to 25 lbs frequently. Close visual acuity including color, depth perception, and field of vision for patient assessment and data transcription. Frequent invasive and non-invasive patient contact; exposure to patient body fluids and hazardous materials. Ability to react quickly to emergency situations and to handle medications, vaccinations, and immunizations as needed.
