Overview
Licensed Practical/Vocational Nurse (LPN/LVN) – Per Diem – Assisted Living at Covenant Living Communities and Services. The role supports quality care in assisted living and memory care.
Responsibilities
Assist routine daily direction and monitor nursing assistants on the unit.
Provide direct patient care including catheter care, colostomy care, dressings, and other treatments.
Observe and evaluate residents’ health, mental and emotional conditions; communicate significant changes to appropriate staff and document actions.
Assist in the admission, orientation, and observation of new residents; transcribe physician orders for diets, therapy, medication and communicate according to procedure.
Participate in shift reports at the beginning and end of shifts to assure proper information about residents is communicated.
Communicate regularly with the Nurse Manager or Director of Assisted Living; provide suggestions of problem areas, resident and employee activities, concerns, needs, conditions, etc.
Assist in the orientation of new LPN/LVNs and nursing assistants and reinforce training standards to maintain a high quality of care.
Provide explanation of nursing procedures and assistance to staff to assure ongoing understanding and training.
When indicated, notify the physician and family members according to prescribed procedures; document notification and follow‑up.
Qualifications
Graduate of an approved school of Licensed Practical/Vocational Nursing.
Current LPN/LVN license in good standing in the State.
One year nursing experience in assisted living or memory care settings.
Devotion to standards and principles of professional nursing.
Strong communication and collaboration skills.
Compensation Pay Range: $36.63 - $45.82 per hour.
