Practice with a multi‑disciplinary team that includes Physicians, Advanced Practice Clinicians, RN Case Managers, Social Workers, and clinic staff. Operate within a comprehensive, coordinated care model for a predominantly Medicare‑aged population. Primarily manage care in the patient's home; some visits may occur in an office or other facility as needed. Diagnose and treat acute and chronic conditions, including diabetes, hypertension, kidney disease and other chronic illnesses. Treat acute conditions such as minor injuries and infections or exacerbations of chronic diseases. Facilitate patient and family decision making regarding treatment and end‑of‑life discussions. Prescribe pharmacological and non‑pharmacological interventions. What makes an Optum Career different?

Base salary plus annual quality bonus with a target of $15,000. 401(k) with match, Executive Savings Plan, and UHG Employee Stock Purchase Program. Required Qualifications

Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or NCCPA. Current licensure through the Texas Board of Nurse Examiners or Texas State Medical Board as a PA. Valid DEA license or ability to obtain one prior to employment. Comfortable managing a primarily Medicare‑aged patient population. 1+ year of clinical experience as a Nurse Practitioner or Physician Assistant. Reliable transportation and a valid driver's license. Preferred Qualifications

Home health experience. Bilingual English/Spanish fluency. Working understanding and support of a managed care environment. The salary range for this role is $104,500 to $156,000 per year, based on full‑time employment and factors such as location, experience, and credentials.
