Principal Responsibilities

Assess physical condition of patient and evaluate patient for graded exercise.

Contact patients in a timely manner after receiving referrals.

Educate patient on what services are available to them.

Document heart/lung sounds, musculoskeletal problems or concerns and status of incision site.

Review medical history to assess problems with beginning physical activity.

Design and implement individualized exercise or activity plans for each assigned patient.

Identify learning needs of each patient.

Report progress on the patient care plan.

Instruct the patient and family on appropriate home exercises/activities.

Monitor progress during exercise sessions.

Analyze and interpret clinical signs and symptoms along with exercise EKG strips during the sessions.

Adjust the exercise plan accordingly.

Follow correct procedures in an emergency situation.

Patient Education.

Assess patient’s family’s knowledge concerning cardiovascular risk factors.

Develop and implement, in collaboration with clients and individuals, realistic education plan.

Conduct classes on heart disease and risk factor modification.

Develop and utilize cardiac rehab educational materials.

Consult/collaborate with physicians and other health professionals regarding patient care, cardiac rehab education and program development.

Communicate with patient’s physician and/or other health care professional as needs dictate.

Participate in meetings with medical director for patient care and quality improvement initiatives.

Maintain department statistics and reports.

Develop ongoing programs to deliver cardiac rehabilitation services.

Maintain equipment.

May set up equipment for each procedure.

May train and provide in-servicing on all outreach accounts on equipment used, including downloading techniques.

May troubleshoot outreach accounts equipment or connectivity issues.

Other duties as assigned.

Required Qualifications

Bachelor's degree in Exercise Physiology, Exercise Science/Kinesiology, Nursing or Occupational Therapy

Preferred Qualifications

Master's degree in Exercise Physiology, Nursing or Occupational Therapy

1 year in cardiac rehab

Licenses/Certifications

Licensed Registered Nurse - MN Board of Nursing required if registered nurse upon hire

BLS Tier 1 - Basic Life Support - Multisource required within 60 Days

ACLS-BLS Tier 2 - Multisource required within 60 Days (excludes Abbott Northwestern Hospital)

Certified Exercise Physiologist preferred

Physical Demands

Medium Work*:

Lifting weight up to 32 lbs. occasionally (*Allina Safe Patient Moving Policy), up to 25 lbs. frequently

0.5 FTE (40-hours per two-week pay period)

8-hour day shifts - (flexible start times between 6:30 -7:30 am)

2 Saturdays a month (based on rotation - details to be discussed during interview)

Pay Range : $30.39 to $41.65 per hour

Pay Range: $30.39 to $41.65 per hour. The pay described reflects the base hiring pay range. Your starting rate would depend on a variety of factors including, but not limited to, your experience, education, and the union agreement (if applicable). Shift, weekend and/or other differentials may be available to increase your pay rate for certain shifts or work.
