Provide primary and/or consultative professional care in various care settings in collaboration with physicians according to a Collaboration Arrangement. Focus on geriatric and seriously ill populations to support patients and families with comfort and quality of life. Secure patient signatures on necessary documents and document in EMR; review HIPAA notices, consents for treatment, release of records, and patient responsibilities forms with patients/caretakers. Drive between locations as required and navigate to each location using GPS or other tools. Assess health status by obtaining medical history and performing age-appropriate physical exams; identify significant health and medical risk factors. Perform Annual Wellness Visits and other quality-driven assessments to provide preventative services and understanding of disease progression. Order, interpret, and evaluate laboratory tests and other studies; develop a diagnosis by analyzing collected data. Perform diagnostic or therapeutic interventions within scope; may prescribe and monitor medications as allowed by prescriptive authority. Develop a treatment plan in collaboration with the patient and family; participate in medical rounds and collaborate with the care team to ensure quality patient care. Refer patients to physicians for consultation or specialized resources; ensure continuity of care as a liaison among care team members. Explain diagnoses and disease progression to patients and families, aiming to reduce unnecessary ER and hospital visits. Required Qualifications
Master's or doctoral degree as a Board-Certified Physician Assistant Current State License as a Physician Assistant Physical capacity to perform duties (sit, stand, bend, lift, move intermittently; ability to lift at least 35 lbs) Current and valid driver's license and reliable transportation/auto insurance Demonstrates skill in PA practice; ability to cope with family emotional stress Ability to manage multiple priorities, documentation requirements, and deadlines Desired Qualifications
2 years clinical experience in a post-acute setting or as a PA Experience with geriatric patients Familiarity with Aprima EMR Location

Office Hours: 8:00 AM – 5:00 PM, Monday through Friday Background Screening
