Diagnoses and treats patients with medical disorders. Delivers a broad range of family medicine services. Promotes ethical and professional standards by providing professional leadership for NHC non-physician providers. Assists in the exploration and research of potential new services and/or the expansion of existing services to improve the services provided. Works cooperatively and in the best interests of NHC, the patient, families, and community. General Summary

Works cooperatively and in the best interests of NHC, the patient, families, and community.
Provides assistance in the provision of orientation, training and in-service education to staff and non-physician providers. Assists in the development, implementation and review of policies and procedures to assure compliance with all regulatory agencies. When requested, assists NHC leadership in establishing productivity targets and collaboratively address variance from targets. Works with the CEO of NHC to ensure safe, effective, and efficient quality of care for patients.

Use evidence-based standards of practice and clinical protocols. Treat patients, families, and team members of NHC with respect and participate in initiatives to improve patient and staff experience with providers. Provide equal access to all patients, including the uninsured. Provide complete, accurate and timely documentation and coding, both for CPT and ICD-10. Facilitate referrals to other health systems and specialty offices as necessary for patient care. Accept accountability for individual and team performance on quality, service, cost-effectiveness, and Lean initiatives. Demonstrate interest/capability in population health management. No employment or ownership in a competing practice or organization. Disclose for review and approval all real or potential conflicts of interest. Participate meaningfully and constructively in Operations meetings (POC and staff). Complete compliance training and comply with all health care laws and regulations. Participate in performance improvement deemed relevant by NHC. Share performance data with NHC providers.

Essential Responsibilities
Treats patients and families at NHC in a primary care setting. Provides wholistic care with other NHC providers, including, but not limited to: mental health, care coordination, and social drivers of health. Responsible for the evaluation and management of medical disorders, including but not limited to: Patient and family education. Outpatient medical consultation at the request of other providers. Provision of care for partial hospitalization and outpatient services as these services develop. Willingly provides consultation for patients to providers and others, as needed. Monitors patient care through clinical supervision of staff and identified treatment teams. Shares in the implementation and review of quality review activities and monitors. Reviews quality reports on a regular basis, provides analysis, conclusions and recommendations. Provides medical education, in-service training, and community education for medical staff, and the general public, as needed. Maintains professional growth and development through continued education as governed by licensing regulations. Performs all evaluations in a competent manner and completes all dictation and clinical documentation in a thorough and competent manner, meeting all requirements of regulatory agencies and NHC requirements. Upholds all current ethical and legal standards and requirements as defined by applicable professional organizations and by applicable regulatory agencies. Understands and practices NHC’s Mission in all interactions with patients, families, visitors, colleagues, and NHC staff. Maintains license in the state of Indiana. Keeps current guidelines on fire/safety hazards and participates in employee health infection control requirements. Completes all NHC-required training. Participates in charitable outreach clinical activities in support of NHC’s mission to support the underserved in the NHC service area. Other duties consistent with the education and expertise required of the position, as determined from time-to-time by your supervisor. Qualifications
Successful completion of NHC orientation and competency-based skills appropriate through ongoing assessment of competence and educational activities. Written evaluations are done on an annual basis.
Education Required:
Graduate of an approved school or college acceptable to the Indiana Professional Licensing Agency.
Experience Required:
Licensed Nurse Practitioner or Physician Assistant with a minimum of three (3) years’ experience in family medicine.
