Home Care Case Manager RN, Minneapolis Team Allina Health is hiring a Home Care Case Manager RN to support in‑home nursing care for patients in the Minneapolis area.

Principal Responsibilities

Comprehensive assessment of patient and family needs; develop care plan and monitor progress.

Maintain education and competencies to provide high quality patient care.

Incorporate evidence‑based nursing practice into care plan.

Coordinate, collaborate, and delegate appropriately to ensure safe, effective care.

Fulfills all organizational requirements.

Other duties as assigned.

Required Qualifications

Associate’s or Vocational degree.

2 or more years RN experience.

Licensed Registered Nurse – MN required upon hire (WI required if hired in WI).

Preferred Qualifications

Bachelor’s degree.

Case Management experience.

Certifications: Certified Hospice and Palliative Nursing (CHPN) preferred; Nursing Case Management Certification (CMGT‑BC) preferred.

Licenses / Certifications

Current BLS certification from the American Heart Association or American Red Cross.

Allina in‑house BLS training within 30 days of hire.

Valid driver’s license required upon hire.

Physical Demands

Lifting weight up to 32 lbs. occasionally; up to 25 lbs. frequently.

1.0 FTE (80 hours per two‑week pay period).

8‑hour day shifts; every 4th weekend.

Pay Range Pay Range: $41.98 to $58.08 per hour. Shift, weekend and/or other differential may be available to increase pay rate.
