Cleveland Clinic Overview: Join the Cleveland Clinic team to provide patient-first healthcare in home settings within the Eastern Cuyahoga area, with potential travel to Lake, Geauga and Ashtabula Counties. The role involves delivering high-quality support and medical services, completing documentation, coordinating visits, and supporting patient care plans. Responsibilities
Provide direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization. Participate in a team-based care model led by the Care Coordinator in each region. Implement and monitor patient care plans established by, and report patient needs after each visit to, the Care Coordinator for further case management. Perform accurate and complete starts of care, resumptions, recertifications and routine visits as requested and within specified time frames. Perform medication reconciliation at each visit. Assess patients for safety risks, abuse and neglect. Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions. Report abnormal findings and/or changes in patient condition to the Physician and update Care Coordinators with those changes and any follow-up needed. Communicate any patient’s missed visits to the Care Coordinator immediately. Participate in and support quality improvement projects and participate in department committees as assigned. Minimum qualifications
Graduate from an accredited School of Professional Nursing Current state licensure as a Registered Nurse (RN) One year of current clinical experience as an RN One year of experience in an acute medical/surgical or homecare setting Valid Ohio Driver's License Proof of automobile insurance coverage ($100,000/$300,000) upon hire and per policy 8-175 thereafter Preferred qualifications

Requires full range of motion, manual and finger dexterity and eye-hand coordination Requires standing and walking for extended periods Requires corrected hearing and vision to normal range Requires some exposure to communicable disease or bodily fluids Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects Must be ambulatory and able to drive to patients' homes, climb stairs, pull, push, and facilitate movement of the patient Must be able to read, write and communicate with patients Personal Protective Equipment
