Primary Function: Provides in-person guidance and support to providers, care teams, and affiliates in clinical integration initiatives that support the quadruple aim and allow the organization to thrive in a value-based health care model.

Key Responsibilities

Conduct in-person and virtual rounds in practice to communicate status reports, key drivers of change, and insights to providers.

Manage review of clinical outcomes, patient panel management, and performance metrics for affiliates and employed physicians.

Partner with providers and care team members to implement evidence-based protocols and other improvement initiatives, monitor progress, document supports, and engage care delivery team.

Conduct provider orientation and facilitate onboarding and offboarding of providers into the assigned network.

Support, encourage, and work closely with providers and care teams to implement process and quality improvement activities necessary for successful participation in value-based agreements.

Collaborate with Regional and practice staff to facilitate market implementation of assigned network initiatives and process improvement activities.

Conduct chart audits for quality composite audits with affiliates not on Epic and chart reviews or quality audits under system or regional clinical leadership.

Facilitate provider remediation tracking and activities in collaboration with assigned network leadership.

Qualifications

Required: BSN (or related) and 5 years of experience in healthcare settings with management or leadership experience in population health or value-based contract work.

Preferred: MSN, MBA, or master’s degree in related field; RN licensure (CCNE, ACEN, or CNEA); experience in value-based arrangements, clinical reporting packages, process improvement models (PDSA, A3).

Location & Remote Work This is primarily a remote/work from home position, but hire must be local to support on-site visits to Toledo, OH market locations (up to 25%).
