Join us as we strive to be The Most Trusted Partner for Health. The Case Manager Patient Care is responsible for all aspects of patient care per an established interdisciplinary plan of care. They collaborate with other health professionals to plan, monitor, and evaluate patient care, requiring population-specific competencies. Qualifications: Associates or Bachelors degree in Nursing Current RN licensure in Oklahoma or multistate license via Nurse Licensure Compact (eNLC) Current CPR certification upon hire or within 30 days Minimum 1 year RN experience; 2 years LPN experience can substitute for one year of RN experience Preferred: 2 years home health experience Valid Oklahoma driver’s license and insurance Effective communication skills in English Must operate INTEGRIS-owned or personal vehicle with acceptable driving record Responsibilities: Perform skilled nursing assessments and implement patient-specific care interventions Manage and coordinate home health services, obtaining treatment authorizations Monitor lab results, notify physicians, and follow up as needed Process physician orders and update care team members Conduct admissions, assessments, recertifications, discharges, and schedule visits Maintain timely documentation per policies Participate in rotating on-call coverage Supervise LPNs and Home Health Aides to ensure quality care Participate in case conferences and quality improvement activities Assist in orientation and training of new staff and students The position reports to the Clinical Manager and may have additional physical or respiratory requirements. Standard precautions must be followed due to potential exposure to infections, blood, body fluids, chemicals, and electrical equipment.
