Responsibilities
Verify the Plan of Care before each visit and provide care according to RN delegation, physician orders, and established standards.
Incorporate patient care goals in each visit, documenting on skill‑nursing notes.
Communicate significant findings and changes in condition to RN Case Manager, physicians, and other care team members.
Report unsafe conditions to the appropriate RN Case Manager and clinical supervisor.
Implement patient safety plans using available resources.
Assist patients in learning self‑care techniques while delivering high‑quality care.
Suggest revisions to the Plan of Care in response to identified issues.
Provide skills under RN or physician direction, documenting medication regimens as required.
Prevent avoidable hospitalizations and ER visits through effective care coordination.
Provide supportive communication to patients, families, and agency personnel.
Qualifications
High School Diploma or GED
Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
Current State License as a Licensed Vocational / Practical Nurse
At least 1 year of experience in a clinical care setting
Experience in a hospice or home health environment preferred
50% travel required
Ability to sit, stand, bend, lift and move intermittently with a capacity to lift 50–100 lbs.
Valid driver’s license, insurance and reliable transportation
