Physical Therapist

WelbeHealth . At WelbeHealth, each participant of our program is guided by an interdisciplinary team (IDT) to provide quality patient-centered care. The Physical Therapist is a key IDT member focusing on care planning and the coordination and delivery of restorative and rehabilitative physical therapy services to participants, working to maximize independence and safety.
This role is distinguished by lower patient volumes, longer-term relationships with participants, collaboration with other therapists and providers, and work-life balance with no after-hours documentation.
Responsibilities
Conduct Physical Therapy assessments in our facility and occasionally in the home to define participant care needs, preferences, and goals
Actively participate in IDT meetings to develop participant care plans
Deliver and document interventions as agreed upon in participant care plans
Provide active and passive exercise, muscle re-education, gait, functional training of ADLs, and treatments including massage, heat, sound, water, light, or electricity
Supervise Therapy Assistants, providing training and guidance to implement Functional Maintenance Programs (FMP)
Provide training and instruction to staff, participants, and caregivers/families on the use and care of therapeutic equipment
Qualifications
Bachelor’s degree in physical therapy; master’s or doctoral degree preferred
Two (2) years in a clinical setting with frail or elderly population preferred
Valid California Physical Therapy License, or proof of license eligibility
Current CPR certification preferred

Compensation Salary/Wage base range for this role is $99,309 - $131,088 per year plus bonus and equity. Actual pay will be adjusted based on experience and other qualifications.
