Provide management and delivery of patient care plans in a home health setting (team-based care). Qualifications
Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college required Licensed Physical Therapist issued by the state in which you will be working Minimum of one year healthcare experience Current driver’s license and ability to spend ~20% of your day driving to/from patient locations Commitment to meet deadlines for charting Ability to self-manage time and schedule Demonstrated experience with tablets, mobile phones and EMR software Compensation & Benefits
