This vital role focuses on assessing patients for hospice eligibility, coordinating their transition to hospice care, and conducting patient admissions. Position Highlights: Serve as a bridge between healthcare providers and hospice patients to ensure compassionate, coordinated transitions of care Conduct in-home and facility-based hospice admissions with clinical accuracy and sensitivity Engage with the community and clinical partners to increase access and awareness of hospice care Collaborate with a team of interdisciplinary professionals committed to quality and comfort What You Will Do: Support the organization’s mission by striving for excellence and fostering positive interpersonal relationships Assess patient needs and provide counseling to patients and families during the hospice referral and admission process Conduct comprehensive hospice admissions, including clinical assessments, eligibility verification, and documentation Coordinate transition to hospice services to promote continuity of care Establish and maintain liaison with physicians, external partners, and internal care teams Monitor and support quality and appropriateness of services and programs Conduct community outreach to enhance hospice awareness and accessibility Assist in the development and implementation of a hospital discharge program at Community Medical Center Adhere to agency infection control and safety protocols and organizational policies What You Will Need: Bachelor’s degree in Nursing required Active New Jersey RN license Excellent clinical skills with experience in hospice and/or community health nursing Experience conducting hospice or homecare admissions strongly preferred Exceptional verbal and written communication abilities Excellent organizational, customer service, and interpersonal skills Strong computer proficiency, including MS Office Suite Valid NJ driver’s license, personal vehicle, and proof of insurance What We Offer: Health, dental, and vision insurance 403B and 401a retirement plans Tuition reimbursement and continuing education programs Robust clinical support and leadership development pathways Employee referral and recognition programs Public Service Loan Forgiveness Partner Working Conditions/Physical Demand:

USD $60,000.00 to USD $110,000.00 .
