Brief Description

Job Objective

:

Under the direction of the Clinical Coordinator, Assistant Director and Director of Pharmacy Services, the Clinical Pharmacist Specialist Pain Management shall: a) provide clinical pharmacy services, including primary and consultative services, to patients and health care providers to ensure optimal drug utilization in all areas, b) assist in the planning and development of clinical pharmacy activities related to pain management and opioid use, c) provide leadership and mentoring for clinical staff to help ensure excellence in clinical pharmacy d) participate in clinical research, oversee and/or participate in the preparation and dispensing of medications, and e) provide education and training to pharmacy students, medical staff physicians and nurses, and healthcare-related professionals. The Clinical Pharmacist Specialist Pain Management will not perform these objectives in an assigned specialty area but will instead work in all areas where pain medications (particularly opioids) are used.

Duties and Responsibilities:

Performs all duties and functions of a Clinical Pharmacist in compliance with all California and Federal pharmacy laws and regulations. Oversee the activities of pharmacy technicians and interns Assists in the orientation and training of departmental personnel in assigned specialty areas as well as other assigned areas of the department. Precepts pharmacy residents, pharmacy graduates, and students from accredited Colleges of Pharmacy in clinical clerkship experiences Maintains professional work habits by organizing workload, setting priorities, completing assignments in a timely manner, utilizing resources appropriately, complying with department program standards and adapting well to change Obtains medication history, maintains and monitors medication profiles Assists with drug formulary management and dissemination of drug information throughout the institution Provides primary care and consultative services to patients and health care providers in assigned specialty area(s) to ensure optimal drug therapy Reviews medication orders to ensure accurate, rational, and cost-effective drug therapy; this includes choice of appropriate drug, indication, route, duration and rate of administration Assesses patient’s drug therapy; performs pharmacokinetic analysis with appropriate adjustments in dosing, and identifies monitoring parameters and therapeutic outcomes in consultation with the patients Develop programs designed to increase pharmacists’ involvement in the promotion of rational drug therapy in the Medical Center; such programs may include pharmacist-regulated drug monitoring protocols, regularly scheduled in-services for Medical Center staff on various drug therapy topics, and improved monitoring and interventions in drug therapy Participates in the development and production of protocols/guidelines, policies and procedures, and other staff development programs for pharmacy or other health care providers Participates in quality focus reviews, adverse drug reaction (ADR) activities, and in the planning, implementation and evaluation of other programs designated by the Pharmacy and Therapeutics Committee or Pain Management Committee Participate in appropriate committees Maintains clinical expertise through consistent literature review, attendance in appropriate seminars, and active membership in state or national pharmacy organizations Participates in on-going clinical research projects and other scholarly activities as appropriate; prepares reports or materials for publication Provides consultation on pain management in patients as identified by prescribers, clinical affiliates and unit-based clinical pharmacists. Help establish best practice for chronic pain management and develop clinical pathways if needed for standardization and optimization of pain regimens. Actively reviews, analyzes, and reports on pain medication use according to class of drug, problem-prone diagnoses, and other opioid and pain related metrics including those defined by the Medical Center and regulatory bodies Review use of opioid reversal agents Coordinates the functioning of the Pain Management Committee by contributing to the agenda and following up on outstanding items Build and support an Opioid Stewardship Program Coordinates Opioid Stewardship (OS) rounds with affected clinical staff

Knowledge

Fully understands and can apply all state and federal laws and regulations related to the practice of pharmacy. Must possess excellent knowledge of current drug therapies, drug interactions, pharmacokinetics, and sociologic principles necessary to assist in the safe and effective utilization of drugs by patients Demonstrates knowledge and proficiency regarding special needs and behaviors of age or specific ethnic groups

Skills

Must possess excellent written and verbal communication skills and demonstrate the ability to collaborate with other health care professionals

Abilities

Ability to adhere to AVMC Policies and Procedures and demonstrate knowledge and understanding of corporate compliance rules and regulations Ability to adhere to safety rules and regulations and maintain the confidentiality of patients, Medical Center, and departmental information Must possess ability to develop and implement medication related protocols

Education and Experience:

Education

Must have Doctor of Pharmacy degree from accredited School of Pharmacy.

Experience

Prefer 2 years of residency (PGY1 and PGY2) or fellowship experience, or equivalent acute care clinical pharmacy experience in specialized area

Required Licensure and/or Certifications:

Must have a current State of California Pharmacist License Board Certified Specialty preferred Completion of a pain management/education certificate through one of the following in the first two years: American Society of Health-System Pharmacists (ASHP) American Society of Pain Educators (ASPE) Other qualifying organization as determined by AVMC Pharmacy management Must have a current BLS card based on the American Heart Association ACLS and PALS certification required if an employee is scheduled to work in related areas of Critical Care, the Emergency Department, Pediatrics, and the NICU

Physical Requirements and Working Conditions:

Works primarily in an office environment. Requires ability to stand, sit or walk around department and facility most of the day. Some bending, lifting less than 20 lbs. and occasionally reaching above the shoulder. Typing, answering phones and working around automated equipment. Potential for exposure to dangerous or toxic medications. May need PPE if preparing chemotherapy.
