The Care Coordinator is a highly visible, customer-service oriented role that works directly with patients, families, insurance representatives, vendors, physicians, clinicians and other medical personnel to provide high-quality healthcare to our members. Responsibilities
Coordinate and process patient referrals to completion with precision, detail and accuracy. Prioritize HPP patients in the Primary Care Physician panel, manage statistics, expedite and place orders when needed (over 5 days). Ensure orders are approved when required; schedule patients (Preferred Providers List of Specialists) and notify them of appointment details (date, time, location). Use Web IVR to generate authorizations (Availity, Careplus, Healthhelp NIA and any other approved web IVR). Complete orders with proper documentation on scheduling status and notification methods. Send referrals to specialist offices and confirm receipt. Prepare for and participate in physician/clinician daily huddles using the RITS Huddle Portal and huddle guide; communicate physicians/clinicians needs or outstanding items regarding patients. Enter all inpatient and outpatient elective procedures in the HITS tool. Reschedule external missed appointments and communicate changes to the physician/clinician. Participate in Super Huddle and provide updates on high-priority patient referrals. Address referral-based phone calls for the Primary Care Physicians panel. Respond to and address phone messages within 24 hours of receipt. Assist with patient checkout when the assigned Care Coordinator is unavailable (note: this does not apply to Care Specialist). Retrieve consultation notes from the consult tracking tool. Follow up on Home Health and DME orders to ensure patients receive services ordered. Provide extraordinary customer service to all internal and external customers (including patients and ChenMed Medical team members). Utilize patient messaging tools as part of daily duties. Perform other related duties as assigned. Qualifications
Must be detail-oriented with the ability to multi-task and be open to cross-functional training in referrals duties. Strong phone etiquette and ability to navigate computer software systems proficiently. Team-oriented with the ability to work well with patients, colleagues, physicians and other personnel in a professional and courteous manner. Exceptional organizational skills with the ability to prioritize and complete tasks in a timely manner. Knowledge of medical terminology, CPT, HCPCS and ICD coding are preferred. High school diploma or equivalent required; up to one year of relevant work experience. Experience with Web IVRs and obtaining referrals/authorizations for multiple payers is strongly preferred. Healthcare experience in a clinical setting, preferably within the Medicare HMO population, is a plus. CPR for Healthcare Providers is preferred. A valid driver’s license in the state of employment is required; travel within the market may be required. Pay
